Appointments are typically available within 1 to 2 weeks, providing convenient access to care from the comfort and privacy of your home.

To request an appointment, please call 386-272-2913 or complete the scheduling form below. Once your request is received, your insurance benefits will be verified and you will be contacted within 24 hours to schedule your appointment. Please note that submitting a request does not guarantee an appointment until it has been confirmed.
A minimum of 24 hours' notice is required to cancel or reschedule an appointment. Missed appointments without notice are subject to a $50 fee. To ensure your appointment can proceed as scheduled, all required paperwork must be completed prior to your visit.
